Transition Planning for Children with a Disability

A major seminar entitled: ‘Transition Planning for Children with a Disability including the Excluded’, is taking place at NISCC on Thursday, 12 April 2018 from 12noon – 2pm.

Presented by Mark Irwin from the Southern Health & Social Care Trust, the seminar will cover the unique service delivery model used within the Trust by the 2018 Social Work Awards Finalists Transition Planning Team.

Key areas covered include:

  • Inter-agency and multi-disciplinary working and challenges
  • Collaborative working and ground breaking work with universities and the voluntary, statutory and private sector
  • Co-production and person centred planning
