You can raise a concern by:

Completing an online referral as an registrant here.

Alternatively, you can download our self-referral form and submit it by email or post.

You are required as part of the Standards of Conduct and Practise to inform us about anything that may impact your fitness to practise.  You should do this at the first reasonable opportunity.  This may include any ill health that affects your ability to practise, criminal convictions, disciplinary proceedings and any findings of other regulatory bodies or organisations.

If you are a social worker, social work student or social care practitioner making a self-referral please notify your employer first before submitting your form.
